De Leon Library wants to hear from you! We need your feedback. This survey is anonymous and open to everyone. Please let us know how your experience has been with De Leon Library, what we're doing well and how we can improve.
Fill out our Community Survey and Giveaway Form for a chance to win a Kindle Fire 7. You do not need a library card to enter the giveaway. YOUR SURVEY ENTRY IS ANONYMOUS AND NOT LINKED TO THE GIVEAWAY FORM.
Limit one entry per person. Open to all community members aged 18+. No purchase required to win. Entries must be submitted by June 30th. Winner will be selected at random from all eligible entries. Winner must pick up their prize in person at De Leon Library. Must provide valid ID to verify identity upon pickup.
